Now that you have been accepted into our Summer Dance Intensive, we need some more information, including required medical forms, a health history, and a photo/video waiver.
A parent or legal guardian must fill out each of the required forms below. Completed forms must be brought in person to registration on the first day of the intensive.
Students are encouraged to download the form below, which includes what to bring (and what not to bring), with you during your stay on campus at IU Bloomington.
