1. Injured graduate student employees must go to either the IU Health Center or Bloomington Hospital Emergency Room. These are the only two emergency services covered by IU insurance.
2. Obtain an Accident Report form from Trish Hausmann, Room M101.
3. The form must be completed in its entirety (incomplete forms will be returned) and returned to Trish Hausmann as soon as possible. All Health Center and Bloomington Hospital invoices should be submitted as soon as possible to the Cindi Severence.

In the event of an accident involving an IU vehicle (department van or truck), please consult the van policy.

Remember: All accidents and damage to the van, no matter how small, must be reported to Trish Hausmann. If they are not reported, the university's insurance does not cover the damage and the department must pay for the repairs. Thank you.
